File Complaint

File a complaint against a trucker driver.

Please include in your complaint the following information:

Name of Trucking Company (Tip: Company name is usually on side of tractor. Sometimes trailer will have different name)

Truck and/or trailer number

Location: Name of Road, Mile Marker, etc. ( Be as specific as possible)

Time and Date

Nature of your complaint ( Tailgating, Speeding, illegal lane change, etc.)
